During frontal impact at speeds of 20MPH, the driver's side air bag did not deploy, and driver's/passenger's side seat belts did not lock.
During a frontal crash the passenger seat belt faild to lock. Passenger was thrown forward in fully extended seatbelt. Passenger sustained facial injuries from hitting the dashboard. Driver also injured. Airbag deployed on driver, and driver's seat belt worked properly.
