2024 Toyota RAV4 forward collision avoidance problems

My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Airbags were not activated. 911 was dispatched. Police report made. Only minor injury finger fracture and chest bruise. There was a severe frontal damage to the car. Insurance deemed it totaled due to frame damage. I also complained to Toyota. They hired a software engineer to investigate.

The contact owns a 2024 Toyota Rav4. The contact stated while driving at an undisclosed speed, the pre-collision warning light illuminated. In addition, the contact stated that the pre-collision warning light illuminated; however, the warning light did eventually go off. The contact stated that the vehicle was taken to the dealer, however the cause of the failure was not determined. The contact was informed that the failure was due to a dirty screen and a rock chip in the windshield.
